Differences in Social Knowledge Between Persons with Intellectual Disability and Persons with Dual Diagnoses

Social knowledge is an important aspect of social cognition that pertains to broader knowledge of social concepts and norms. People with intellectual disabilities are more likely to experience mental health challenges, and it’s important to pay special attention to how comorbid conditions can affect their social cognition skills, potentially weakening these skills. Consequently, the present study seeks to compare social knowledge between two groups of adults in Bosnia and Herzegovina: those with intellectual disabilities and those diagnosed with a dual diagnosis encompassing intellectual disability and psychiatric conditions. An additional goal was to identify the factors contributing to social knowledge in these groups.
Methods
The study sample included 62 adults with mild intellectual disability, divided into two groups based on their comorbid psychiatric condition. We used a demographic questionnaire, Raven’s Progressive Matrices, Peabody Picture Vocabulary Test (PPVT), MINI PAS – ADD scale, and The Social Knowledge Test to assess social knowledge and intellectual functioning. We compared the social knowledge scores between the two groups and identified the predictors of social knowledge in each group.
Results
The results indicated that adult participants with dual diagnoses had lower social knowledge scores than those with intellectual disabilities only, even after controlling for intellectual functioning and verbal abilities. The predictors of social knowledge differed between the two groups, with age being the only statistically significant predictor in both groups. In individuals with intellectual disabilities, age and the presence of obsessive-compulsive disorder symptoms were important predictors of social knowledge, while in the group of participants with dual diagnoses, age and PPVT were significant predictors of social knowledge.
Conclusion
This study highlights the importance of social knowledge in individuals with intellectual disabilities and dual diagnoses. The findings suggest that individuals with dual diagnoses may have a specific deficit in social knowledge that is not fully explained by their intellectual functioning or verbal abilities. Clinicians and educators should focus on identifying and addressing social knowledge deficits in individuals with dual diagnoses to improve their overall social functioning.

Iskustva sa programom „Trening veština roditelja“ u Srbiji.

AB  - U poslednjih nekoliko godina, dolazi do značajne promene paradigme u pogledu programa intervencija za
decu sa teškoćama u razvoju. Navedeni programi se sa tradicionalnog medicinskog modela pomeraju da
modele prilagođene individualnim karakteristikama deteta sa problemom u razvoju i njihovom okruženju,
ali istovremeno i samim roditeljima/starateljima i celokupnim porodicama. Istraživanja u različitim
zemljama sveta pokazuju da „Trening veština roditelja“ (engl. Caregivers’ Skills Training - CST) može
postati jedan od značajnijih programa u širokoj paleti intervencija. Program pokazuje značajne rezultate u
sferi razvojnih intervencija zbog lake implementacije, široke upotrebe, svog sveobuhvatnog i praktičnog
pristupa starateljima koji kroz program stiču veštine potrebne za rad sa sopstvenom decom kako bi im
adekvatno pružali razvojnu podršku. Od implementacije programa u Srbiji, program je prošao kroz dve
konsekutivne faze, pre-pilot i pilot. Kroz ove dve faze obučeno je ukupno 24 profesionalaca i 36 porodica
dece sa problemima u razvoju u četiri grada u Srbiji. Tokom perioda obuke, roditelji su popunjavali
upitnike procene razvoja deteta (ATEC) na početku i kraju programa, kao i upitnike zadovoljstva
sesijama i samim programom. Statistička analiza je pokazala da je došlo do značajnog poboljšanja u
domenima govora, jezika i komunikacije (z=-2.99, p˂0.05), ponašanja i opšteg telesnog zdravlja (z=6.00,
p˂0.01) dece nakon pohađanja ovog dvanaestonedeljnog programa. Reakcije roditelja prikupljane nakon
samih sesija su bile dominantno pozitivne. Preliminarni rezultati programa “Treninga veština roditelja” u
našoj zemlji su ohrabrujući. Da bi ovaj program postao preporučena intervencija za populaciju dece sa
poteškoćama u razvoju zasnovana na dokazima, potrebna su dodatna istraživanja na većim uzorcima.

Epilepsy and interictal epileptiform activity in patients with autism spectrum disorders

AB  - Purpose: The purpose of this study was to determine the prevalence of epilepsy and subclinical epileptiform abnormalities in children with autism spectrum disorder (ASD), and to investigate its effects on core autistic symptoms and adaptive behavior skills. Methods: Patients with diagnosis of ASD who met full criteria on Autism Diagnostic Interview-Revised (ADI-R) were included in the study. Adaptive behavior skills were assessed by Vineland Adaptive Behavior Scale-II (VABS-II). Clinical assessment for epilepsy and video electroencephalography (EEG) (v-EEG) examinations during wakefulness and/or sleep were prospectively performed in all patients. Results: A total of 112 patients with diagnosis of ASD of mean age 6.58 +/- 3.72 were included in the study. Based on clinical and v-EEG assessments, three groups of patients were defined: 1) patients with epilepsy (n = 17; 15.2%); 2) patients with epileptiform discharges in absence of clinical seizures (n = 14; 12.5%); 3) patients without epilepsy and without epileptiform discharges ( n = 81; 72.3%). There were no significant differences between three groups of patients on ADI-R subscores. Speech development was also not significantly related to epilepsy. There was a slight tendency of the VABS-II motor skills score to be higher in the group of patients with autism without clinical diagnosis of epilepsy and without subclinical epileptiform discharges (p  lt  0.05) in comparison with the two other groups. According to this tendency, we might claim that patients with higher scores on motor skills could have 0.88 times lower odds for having epileptiform EEG activity. Conclusions: According to our results, we were not able to detect differences in the ADI-R between the three populations with ASD, all with unknown etiology. Epilepsy, as well as subdinical epileptic discharges, showed small effects on Motor Skills in patients with autism, and had no effect on adaptive behavior Communication/Socialization/Daily Living Skills.

AB  - Early detection of Autism Spectrum Disorder (ASD) has proven to be of high significance, however there is a limited availability of ASD screening tools in Serbian language. In this study we aim to translate, assess reliability and, in part, test the applicability of Modified Checklist for Autism in Toddlers, Revised, with Follow-Up (M-CHAT R/F) in Serbian Healthcare environment. We screened 128 children in three primary healthcare centres and 20 children in a tertiary psychiatric center, using M-CHAT R/F translated into Serbian language, between December 2014 and October 2015. At the end of the screening process 80% of participants in the risk group screened positive for ASD, while in the control group 4 (3.1%) participants screened positive, with a mean total scores of 8.25 and 0.66 respectively. The Cronbach's a coefficient was 0.91 and Guttman's.6 was 0.93. Test-retest reliability was deemed as acceptable, and no significant correlation was found between M-CHAT-R/F scores and Epworth Sleepiness Scale for children scores. The Serbian version of the M-CHAT-R/F has shown satisfactory reliability. We can therefore assert that it is a reliable tool for identifying ASD and it can be used in clinical practice to improve early detection, assessment and treatment.

Attention-deficit/hyperactivity disorder: Prevalence of symptoms in lower elementary school children in Serbia

AB  - Introduction: Attention-deficit/Hyperactivity Disorder is the most common neurobehavioral disorder in childhood. It is characterized by symptoms of inattention, impulsivity and hyperactivity that can vary in intensity and develop a chronic course. The prevalence of ADHD in school children is 1-20%, and male-female ratio is 3:1. The aim of this study was to determine prevalence, age and sex distribution of symptoms of ADHD on a sample of school children from 3 elementary schools in Serbia, and to establish possible differences in these parameters between children in the capital city and in a small town. Method: Our study included 376 respondents from first to fourth grade from two elementary schools in center Belgrade and one school in Arilje. The study was conducted using IOWA Conners rating scale that was filled by teachers for every child in their class. Results: Score equal or above 15 was present in 8.51% of children and male-female ratio was 2.55:1. Score equal or above 20 was present in 3.72% of children, and male-female ratio regarding the greater score, i.e. more expressed symptoms was 13:1. Conclusion: The results of our study suggest that the symptoms of ADHD are significantly present in Serbia, and that it is of great importance to educate teachers and parents to recognize ADHD and to actively participate in the therapeutical process of a child with this disorder.
AB  - Hiperkinetski poremećaj sa poremećajem pažnje (ADHD - Attention- Deficit/Hyperactivity Disorder) predstavlja najčešći neurobihejvioralni poremećaj u detinjstvu. Karakteriše se simptomima nepažnje, impulsivnosti i hiperaktivnosti koji mogu varirati po intenzitetu i imati hronični tok. Učestalost ADHD kod školske dece iznosi 1-20%, a zastupljenost poremećaja u odnosu na pol 3:1 u korist muškog pola. Cilj: Cilj ovog rada bio je određivanje učestalosti, uzrasne i polne distribucije simptoma ADHD-a na uzorku školske dece tri osnovne škole u Srbiji i ispitivanje eventualnih razlika u ovim parametrima između dece iz glavnog grada i manjeg mesta. Metod: Istraživanje je obuhvatilo 376 ispitanka od prvog do četvrtog razreda dve osnovne škole iz centra Beograda i jedne osnovne škole iz Arilja. Istraživanje je sprovedeno primenom IOWA Conners rating skale koju su popunjavali učitelji za svako dete iz svog odeljenja. Rezultati: Skor jednak ili veći od 15 imalo je 8.51% dece a odnos muškog i ženskog pola bio je 2.55:1 u korist dečaka. Skor jednak ili veći od 20 imalo je 3.72% dece, a odnos muškog i ženskog pola kada je u pitanju veći skor, odnosno jače izraženi simptomi, iznosio je 13:1 u korist muškog pola. Zaključak: Rezultati našeg istraživanja pokazuju da je ADHD često prisutan u našoj sredini, i da je zbog toga je od velike važnosti obrazovati nastavnike i roditelje o ADHD-u, kako bi mogli da ga prepoznaju i aktivno učestvuju u terapijskom postupku deteta sa ovim poremećajem.
