Prioriteti u tretmanu dece sa poremećajem iz spektra autizma – perspektiva roditelja

Abstract

Приоритети родитеља важан су критеријум приликом конципирања и имплементације плана третмана за децу са поремећајем из спектра аутизма (ПСА). Циљеви овог истраживања јесу да се утврде родитељски приоритети у третману деце са ПСА, али и постојање везе између њих и нивоа функционисања детета и третмана у који се дете укључује, као и утврђивање разлике у приоритетима родитеља у односу на узраст детета. Узорак је чинило 100 родитеља, 85 мајки и 15 очева, старости од 21 до 65 година. Коришћен је општи упитник за прикупљање социо-демографских података испитаника и детета са ПСА и Инструмент за испитивање родитељских приоритета (Parent Priorities Survey, Pituch et al., 2011). Као највише приоритете родитељи перципирају следеће вештине: Иницира и тражи интеракцију са другима (област Социјализација), Сналази се у саобраћају као пешак (област Активности живота у заједници), Прикладно комуницира са непознатим особама (област Социјализација), Именује објекте и Изражава жеље и потребе (област Комуникација), Лична безбедност (област Активности живота у заједници), Цртање и бојење (област Академске вештине), Ствара пријатељства (област Социјализација), Прати инструкције (област Академске вештине), Игра се са вршњацима (област Слободно време и рекреација), Пита за информације и Започиње конверзацију (област Комуникација), Читање, Писање и Рачунање (област Академске вештине), Уме да описује догађаје и осећања и Прикладно реагује на постављена питања (област Комуникација), Слуша наставника (област Академске вештине). Пронађене су статистички значајне корелације између процењеног нивоа способности и приоритета родитеља, изузев корелација које се односе на област Академске вештине. Наиме, уколико су родитељи перципирали да су деца мање способна за самостално обављање одређених вештина, уједно су процењивали да је одређена вештина за њих приоритетнија. Поред тога, пронађена је и статистички значајна позитивна корелација између већине третмана и приоритета родитеља. Уколико су од стране родитеља одређена понашања процењена као приоритетнија, постојала је велика вероватноћа да су она и третирана. Добар део третмана деце са ПСА није усмерен на процењене приоритетне вештине, па је неопходно разумевање чинилаца који утичу на одлуку о избору третмана, како би се, од стране стручњака, пружиле потребне информације, помоћ и подршка родитељима, не само у одабиру, већ и у истрајности у спровођењу временски захтевних интервенција. Резултати би омогућили унапређење положаја родитеља деце са ПСА, побољшање квалитета подршке која им се пружа и могућност осмишљавања третмана и програма у складу са њиховим потребама.

Parents’ priorities are an important criterion when designing and implementing a treatment plan for children with autism spectrum disorder (ASD). The goals of this research are not only to determine parental priorities in the treatment of children with ASD, but also the existence of a relation between them and the level of functioning of the child and the treatment that the child is currently relation, as well as determining the difference in parents’ priorities in relation to the child's age. The sample consisted of 100 parents, 85 mothers and 15 fathers, aged 21 to 65 years. A general questionnaire was used to collect socio-demographic data of the respondents and the child with ASD and the Parent Priorities Survey (Pituch et al., 2011). Parents perceive the following skills as the highest priorities: Initiates and seeks interaction with others (Socialization area), Participates in traffic as a pedestrian (Community life activities area), Communicates appropriately with strangers (Socialization area), Names objects and Expresses wants and needs (Communication area), Personal safety (Community living activities area), Drawing and coloring (Academic skills area), Forms friendships (Socialization area), Follows instructions (Academic skills area), Plays with peers (Leisure and recreation area), Asks for information and Starts a conversation (Communication area), Reading, Writing and Numeracy (Academic skills area), Can describe events and feelings and Responds appropriately to questions (Communication area), Listens to the teacher (Academic skills area). Statistically significant correlations were found between the estimated level of ability and parents’ priorities, except for correlations related to the area of Academic Skills. Namely, if the parents perceived that the children were less capable of performing certain skills independently, they also estimated that a certain skill was a higher priority for them. In addition, a statistically significant positive correlation was found between most treatments and parents' priorities. If the parents rated certain behaviors as a higher priority, there was a high probability that they were treated. A good part of the treatment of children with ASD is not aimed at the assessed priority skills, so it is necessary to understand the factors that influence the decision to choose certain treatment, so that the experts can provide the necessary information, help and support to parents, not only in the selection, but also in persistence in implementing time-consuming interventions. The results would enable the improvement of the position of parents of children with ASD, the improvement of the quality of support provided to them and the possibility of designing treatments and programs in accordance with their needs.
