dc.description.abstractSprovedeno istraživanje je usmereno na otkrivanje prediktora uspešnostirehabilitacije govora kod laringektomiranih bolesnika.Cilj istraživanja je da se ispita povezanost između akustičkih i perceptualnihparametara, kao i razumljivosti govora kod osoba sa laringektomijom, koji koristerazličite oblike govorne komunikacije: ezofagealni govor, govor uz pomoćtraheoezofagealne vokalne proteze i govor uz pomoć elektronske laringealne proteze.Krajnji cilj istraživanja bio je da se utvrdi koji je od ta tri oblika govorne komunikacijenajefikasniji, odnosno najrazumljiviji u svakodnevnoj komunikaciji.Istraživanje je izvršeno na uzorku od 125 laringektomiranih bolesnika, oba pola,starosne dobi od 48 do 85 godina.Instrumenti koji su bili korišćeni u ovom istraživanju su: a) Kompjuterizovanalaboratorija za akustičku analizu, “Kay Elemetrics“ korporacije, model 4300, kojom smoispitivali akustičke parametre glasa i govora–parametre varijabilnosti frekvencije (Fo,To, PFR, Jita, Jitt, RAP, PPQ), parametre varijabilnosti intenziteta (ShdB, Shim, APQ,vAm), parametre procene šuma i tremora u glasu (NHR, VTI, SPI), parametar MPT–maksimalno vreme fonacije vokala, parametar WPM–broj izgovorenih reči u minuti,parametre spektralne analize govora–formantna struktura samoglasnika i suglasnika; b)GRBAS skala za perceptualnu procenu kvaliteta glasa; v) Test za procenu razumljivostigovora.U statističkoj obradi dobijenih podataka korišćene su: mere centralne tendencije(aritmetička sredina), mere varijabilnosti (standardna devijacija), jednofaktorskaanaliza varijanse, t-test za nezavisne uzorke, Spirmanov koeficijent korelacije ivišestruka linearna regresija.Najbolje rezultate na testovima akustičke i perceptualne procene glasa, kao irazumljivosti govora ostvarili su pacijenti koji koriste traheoezofagealne vokalneproteze. Kod pacijenata sa ezofagealnim govorom, parametri procene hrapavosti i slabostiu glasu, GRBAS skale, pokazali su se kao najznačajniji prediktori razumljivosti govora(β=0,000; R²=0,715; F=26,10, p=0,000). Kod pacijenata koji koriste vokalne proteze,parametar procene napetosti u glasu, GRBAS skale, predstavlja najznačajniji prediktorrazumljivosti govora (β=0,000; R²=0, 458; F=18,33, p=0,000).sr
dc.description.abstractConducted research is directed to identify predictors of success in speech rehabilitation oflaryngectomized patients.The aim of research is to examin connection between acoustic and perceptive parameters aswell as speech intelligibility of laryngectomized persons that use different types of vocalcommunication: esophageal speech, speech with tracheoesophageal prosthesis and speech withelectonic laringeal prosthesis. The final aim of this research was to determine which of the threementioned vocal communication types is the most effective and the most intelligible in everydaycommunication.The research was done on the sample of 125 laryngectomized patients of both sexes, from48 to 85 years old. Instruments that were used in this research are: a) Computer laboratory foracoustic analysis, of «Kay Elemetrics» Corporation model 4300 to examin acoustic parameters ofvoice and speech –parameters of frequency variation (Fo, To, PFR, Jita, Jitt, RAP, PPQ),parameters of intensity variation (ShdB, Shim, APQ, vAm), assessment parameters of voicebreathiness and strain), MPT parameter - maximum phonation time of vocal, WPM – number ofwords pronounced per minute, speech spectral analysis parameters – formant structure of vowelsand consonants; b) GRBAS scale for a perceptive evaluation of voice quality; c) Speechintelligibility test.In statistical data analysis we used: measures of central tendency (arithmetic mean),measures of variability (standard deviation), one factor analysis of variance, t-test for independentsemples, Spearman's correlation coefficient and Multiple linear regression.Patients that use tracheoesophageal vocal prosthesis had the best achievement on acousticand perceptual voice assesment tests as well as on intelligibility test. Parametar of voice roughnessand asthenia of GRBAS scale were the most significant predictors of speech intelligibility ofpatients with esophageal speech (β=0,000; R²=0,715; F=26,10, p=0,000). Parameter of strain invoice of GRBAS scale was the most significant predictor of speech intelligibility of patients withvocal prosthesis (β=0,000; R²=0, 458; F=18,33, p=0,000). On the subsemple of patients withelectrolarynx parameter of voice hoarsness of GRBAS scale was the most significant predictor ofspeech intelligibility (β=0,004; R²=0,399; F=7,46, p=0,000).en
