Association of GSTO1, GSTO2, GSTP1, GPX1 and SOD2 polymorphism with primary open angle glaucoma

AB  - It is becoming increasingly evident that oxidative stress has a supporting role in pathophysiology and progression
of primary open angle glaucoma (POAG). The aim of our study was to assess the association between polymorphisms
in genes encoding enzymes involved in redox homeostasis, mitochondrial superoxide dismutase
(SOD2), glutathione peroxidase (GPX1) and glutathione transferases (GSTs) with susceptibility to POAG. Single
nucleotide polymorphisms in GST omega (GSTO1rs4925, GSTO2 rs156697), pi 1 (GSTP1 rs1695), as well as
GPX1 (rs1050450) and SOD2 (rs4880) were determined by quantitative polymerase chain reaction (qPCR) in 102
POAG patients and 302 respective controls. The risk for POAG development was noted in carriers of both
GSTO2*GG and GSTO1*AA variant genotypes (OR = 8.21, p = 0.002). Individuals who carried GPX1*TT and
SOD2*CC genotypes had also an increased risk of POAG development but without significance after Bonferroni
multiple test correction (OR = 6.66, p = 0.005). The present study supports the hypothesis that in combination,
GSTO1/GSTO2, modulate the risk of primary open angle glaucoma.

AB  - Vitamin B1 (aneurin, thiamine) is a water-soluble vitamin necessary for the normal function of the nervous
system, visual system and heart and is part of important enzymes in the body. Thiamine enables the normal
use of glucose, other carbohydrates and proteins, and enables the supply of energy to the organism.
The main sources of thiamine are exogenous and small amounts are synthesized by microorganisms of
the human intestinal microbiome.
Vitamin B1 cannot accumulate in the body, so signs of deficiency are quickly manifested. Hypovitaminosis
B1 is seen in chronic ethyl abuse, persistent vomiting (as in some pregnant women) or after bariatric
surgical procedures, but in a mild form it is present in the general population.
Normal daily needs for vitamin B1 depend on calorie intake, and 0.4 mg should be ingested for every
1000 kcal.

dan je za normalno funkcionisanje nervnog sistema, organa
vida i srca i deo je važnih enzima u telu. Tiamin omogućava
normalnu upotrebu glukoze, drugih ugljenih hidrata i pro-
teina i snabdevanje organizma energijom.
Glavni izvori tiamina su egzogeni i male količine sinte-
tišu mikroorganizmi ljudskog crevnog mikrobioma.

Outcomes of Early Correction of Congenital Myogenic Ptosis Using Transconjunctival Levator Plication

AB  - Introduction: Early correction of congenital ptosis may be indicated due to a risk of amblyopia or because of an abnormal head tilt. One of the main problems, of planning ptosis surgery in very young children, is the inability to measure the levator function. Aim: The aim of the article was to analyze the early correction of congenital myogenic ptosis. Methods: This was a retrospective, interventional, case series study, conducted on 12 eyes of 12 patients with unilateral, mild to moderate, congenital myogenic ptosis. Surgical correction of ptosis was performed by transconjunctival levator muscle plication. Pre- and postoperative measurements of the upper lid margin to central corneal reflex (MRD1) and upper lid skin crease height (UEC) were obtained, as well as the presence or absence of a reaction to topically applied phenylephrine 2.5% solution. Results: The mean age of the patients was 29.83 months (range 14-45 months). A negative phenylephrine test was noted in only 3 (25%) of cases. Equalization of upper lid height was achieved in 6 (50%), and a hypocorrection of up to 1 mm was noted in 4 (33%) of patients. There was only 1 hypercorrection of 1 mm, noted in the first postoperative month. In one case of hypocorrection of 2 mm, the height of the lid dropped between the 1 and 3 months follow up. Subsequent revision surgery was performed, with a good outcome. With regard to the upper lid skin crease height (UEC), the mean preoperative difference in relation to the contralateral (non-operated) lid, was 2.16 mm, whereas the average postoperative or final difference was 0.41 mm. Conclusion: Correction of myogenic ptosis in small children, using transconjunctival levator plication, in whom levator function cannot be measured, may have a satisfactory postoperative outcome.

Relationship between optic nerve head topography and nerve fiber layer thickness with central corneal thickness in patients with primary open-angle glaucoma

AB  - Introduction/Objective In patients with primary open-angle glaucoma (POAG) we explored the relationship between the optic nerve head (ONH) topography parameters and the retinal nerve fiber layer (RNFL) thickness with the central corneal thickness (CCT). Methods This retrospective study included 97 patients (97 eyes) with primary open-angle glaucoma. Patients were divided into a thin CCT  lt  540 mm (45 eyes) and a thick CCT ≥ 540 mm (52 eyes) group, using ultrasonic pachymeter. Topographic measurements of the ONH parameters and RNFL thickness was performed using optical coherence tomography (OCT). The outcomes were compared with the thin and thick CCT and correlated with the thin CCT of the subjects. Results There were significantly lower mean intraocular pressure (p  lt  0.0001) and CCT (p  lt  0.0001) in patients with thin CCT compared to patients with thick CCT. Statistically significant differences of ONH parameters were found in thin cornea group compared to thick cornea group in: cup/disc area ratio (p  lt  0.03), vertical cup/disc ratio (p  lt  0.01) and rim volume (p  lt  0.01). Statistically significant differences of RNFL thickness were found in thin cornea group compared to thick cornea group in: average (p  lt  0.001), superior (p  lt  0.03), inferior (p  lt  0.03) and nasal (p  lt  0.01). Significant positive correlation was found between thin CCT and OCT parameters in: optic disc area (r = 0.429, p = 0.003), cup/disc area ratio (r = 0.287, p = 0.05), horizontal cup/disc ratio (r = 0.472, p  lt  0.001), vertical cup/disc ratio (r = 0.578, p  lt  0.001), average RNFL (r = 0.796, p  lt  0.001), superior RNFL (r = 0.665, p  lt  0.001), inferior RNFL (r = 0.650, p  lt  0.001), nasal RNFL (r = 0.611, p  lt  0.001) and temporal RNFL thickness (r = 0.601, p  lt  0.001). Conclusion POAG patients with thin cornea will probably develop larger glaucoma changes than those with a thicker cornea. Ultrasonic pachymetry measurements of CCT and OCT analysis of ONH topography parameters and RNFL thickness provide significant information in early diagnosis and monitoring progression of POAG.
AB  - Uvod/Cilj Istraživali smo povezanost između topografskih parametara glave optičkog nerva (GON) i debljine sloja nervnih vlakana retine (SNVR) sa centralnom debljinom rožnjače (CDR) kod bolesnika sa primarnim glaukomom otvorenog ugla. Metode U ovu retrospektivnu studiju uključeno je 97 bolesnika (97 očiju) sa primarnim glaukomom otvorenog ugla. Bolesnici su podeljeni na grupu sa tanjom rožnjačom (CDR  lt  540 mm, 45 očiju) i debljom rožnjačom (CDR ≥ 540 mm, 52 oka), koje su merene ultrazvučnom pahimetrijom. Topografska merenja parametara GON i debljine SNVR rađena su optičkom koherentnom tomografijom (OKT). Rezultati su upoređivani sa tanjom i debljom CDR i korelirani sa tanjom CDR učesnika studije. Rezultati Utvrđene su statistički značajno niže vrednosti intraokularnog pritiska (p  lt  0,0001) i CDR (p  lt  0,0001) kod bolesnika sa tankom CDR u poređenju sa bolesnicima sa debljom CDR. Kod parametara GON dobili smo statistički značajnu razliku u grupi sa tankom rožnjačom u poređenju sa grupom sa debljom rožnjačom kod površine odnosa cup/disc (p  lt  0,03), vertikalnog odnosa cup/disc (p  lt  0,01) i volumena rim (p  lt  0,01). Pronađena je statistički značajna razlika kod debljine SNVR u grupi sa tankom rožnjačom u poređenju sa grupom sa debljom rožnjačom kod srednje (p  lt  0,001), gornje (p  lt  0,03), donje (p  lt  0,03) i unutrašnje (p  lt  0,01) debljine SNVR. Utvrđena je statistički značajna pozitivna korelacija tanke CDR i parametara optičke koherentne tomografije kod površine optic disc (r = 0,429, p = 0,003), površine odnosa cup/disc (r = 0,287, p = 0,05), horizontalnog odnosa cup/disc (r = 0,472, p  lt  0,001), vertikalnog odnosa cup/disc (r = 0,578, p  lt  0,001), srednje debljine SNVR (r = 0,796, p  lt  0,001), gornjeg SNVR (r = 0,665, p  lt  0,001), donjeg SNVR (r = 0,650, p  lt  0,001), unutrašnjeg SNVR (r = 0,611, p  lt  0,001) i spoljašnjeg SNVR kvadranta (r = 0,601, p  lt  0,001). Zaključak Bolesnici sa primarnim glaukomom otvorenog ugla i tanjom rožnjačom verovatno će razviti veće glaukomne promene od onih sa debljom rožnjačom. Ultrazvučnom pahimetrijom merena CDR i optičkom koherentnom tomografijom analizirani topografski parametri GON i debljina SNVR pružaju značajne informacije u ranoj dijagnozi i praćenju progresije kod primarnog glaukoma otvorenog ugla.

Significant positive correlation was found between thin CCT and OCT parameters in: optic disc area (r = 0.429, p = 0.003), cup/disc area ratio (r = 0.287, p = 0.05), horizontal cup/disc ratio (r = 0.472, p  lt  0.001), vertical cup/disc ratio (r = 0.578, p  lt  0.001), average RNFL (r = 0.796, p  lt  0.001), superior RNFL (r = 0.665, p  lt  0.001), inferior RNFL (r = 0.650, p  lt  0.001), nasal RNFL (r = 0.611, p  lt  0.001) and temporal RNFL thickness (r = 0.601, p  lt  0.001). Conclusion POAG patients with thin cornea will probably develop larger glaucoma changes than those with a thicker cornea. Ultrasonic pachymetry measurements of CCT and OCT analysis of ONH topography parameters and RNFL thickness provide significant information in early diagnosis and monitoring progression of POAG., Uvod/Cilj Istraživali smo povezanost između topografskih parametara glave optičkog nerva (GON) i debljine sloja nervnih vlakana retine (SNVR) sa centralnom debljinom rožnjače (CDR) kod bolesnika sa primarnim glaukomom otvorenog ugla. Metode U ovu retrospektivnu studiju uključeno je 97 bolesnika (97 očiju) sa primarnim glaukomom otvorenog ugla. Bolesnici su podeljeni na grupu sa tanjom rožnjačom (CDR  lt  540 mm, 45 očiju) i debljom rožnjačom (CDR ≥ 540 mm, 52 oka), koje su merene ultrazvučnom pahimetrijom. Topografska merenja parametara GON i debljine SNVR rađena su optičkom koherentnom tomografijom (OKT). Rezultati su upoređivani sa tanjom i debljom CDR i korelirani sa tanjom CDR učesnika studije. Rezultati Utvrđene su statistički značajno niže vrednosti intraokularnog pritiska (p  lt  0,0001) i CDR (p  lt  0,0001) kod bolesnika sa tankom CDR u poređenju sa bolesnicima sa debljom CDR. Kod parametara GON dobili smo statistički značajnu razliku u grupi sa tankom rožnjačom u poređenju sa grupom sa debljom rožnjačom kod površine odnosa cup/disc (p  lt  0,03), vertikalnog odnosa cup/disc (p  lt  0,01) i volumena rim (p  lt  0,01). Pronađena je statistički značajna razlika kod debljine SNVR u grupi sa tankom rožnjačom u poređenju sa grupom sa debljom rožnjačom kod srednje (p  lt  0,001), gornje (p  lt  0,03), donje (p  lt  0,03) i unutrašnje (p  lt  0,01) debljine SNVR. Utvrđena je statistički značajna pozitivna korelacija tanke CDR i parametara optičke koherentne tomografije kod površine optic disc (r = 0,429, p = 0,003), površine odnosa cup/disc (r = 0,287, p = 0,05), horizontalnog odnosa cup/disc (r = 0,472, p  lt  0,001), vertikalnog odnosa cup/disc (r = 0,578, p  lt  0,001), srednje debljine SNVR (r = 0,796, p  lt  0,001), gornjeg SNVR (r = 0,665, p  lt  0,001), donjeg SNVR (r = 0,650, p  lt  0,001), unutrašnjeg SNVR (r = 0,611, p  lt  0,001) i spoljašnjeg SNVR kvadranta (r = 0,601, p  lt  0,001). Zaključak Bolesnici sa primarnim glaukomom otvorenog ugla i tanjom rožnjačom verovatno će razviti veće glaukomne promene od onih sa debljom rožnjačom. Ultrazvučnom pahimetrijom merena CDR i optičkom koherentnom tomografijom analizirani topografski parametri GON i debljina SNVR pružaju značajne informacije u ranoj dijagnozi i praćenju progresije kod primarnog glaukoma otvorenog ugla.",

AB  - Purpose The study aim was to translate and validate the Glaucoma Symptom Scale (GSS) in Serbian language. Methods Clinical parameters and socio-demographic data were collected for each of the 177 enrolled glaucoma patients. Each eye was classified according to the Glaucoma staging system by Mills into 6 stages. Patients filled out the GSS and National Eye Institute Visual Function Questionnaire (NEI-VFQ 25). The GSS comprises 10 complaints common for glaucoma patients on a topical treatment, grouped into two subscales: SYMP-6 (non-visual) and FUNC-4 (visual problems). The GSS was translated following the customary methodology and its psychometric properties were assessed by using both Classical Test Theory (CTT) and Rasch analysis. Results The internal consistency of the Serbian GSS for the whole scale was very good (Cronbach's alpha = 0.81). On factor analysis items were clustered into 2 factors (48.92% of variance) which corresponded to the original scale. The total and subscale GSS scores correlated significantly with measures of disease severity and also with total score and analogous NEI-VFQ 25 subscale scores. In Rasch analysis we obtained adequate item reliability index (0.90). Almost all items had infit and outfit mean squares in the accepted range. However, measurement precision was poor (low person separation reliability) and targeting revealed a ceiling effect. Conclusion When analyzed with CTT the Serbian version of the GSS seems to be a valid instrument, but Rasch analysis revealed some serious measurement flaws, therefore it should not be used in its current format. Further studies to modify and improve GSS are needed prior to its application for Serbian glaucoma patients.

AB  - AIM: To translate the Glaucoma Quality of Life-15 (GQL-15) to Serbian language and asses its validity and reliability in the population of Serbian patients. METHODS: The study included 177 glaucoma patients. Clinical parameters (visual acuity, mean defect and square root of loss variance of visual field) and socio-demographic data were collected. Patients were stratified according to the Nelson's glaucoma staging system as mild, moderate and advanced glaucoma. All patients filled out the GQL-15 and National Eye Institute Visual Function Questionnaire (NEI-VFQ 25). The GQL-15 was translated following the internationally-accepted methodology, and its psychometric properties were assessed by using classical test theory and Rasch analysis. RESULTS: The mean total score for the GQL-15 was 20.68 +/- 7.31. The Cronbach's alpha coefficient for the whole scale was 0.89 (central and near vision, alpha=0.24; peripheral vision, alpha=0.85; glare and dark adaptation, alpha=0.83). Factor analysis established 4 factors (70.3% of variance): two corresponding to the original factors and two new factors specific for the Serbian population. The GQL-15 score correlated positively with almost all clinical parameters and NEI-VFQ 25 proving good criterion validity. Correlation of the GQL-15 total score on test-retest confirmed appropriate scale reproducibility (rho= 0.96, P lt 0.001). The GQL-15 discriminated well advanced from mild and moderate glaucoma. In Rasch analysis we obtained adequate item (0.95) reliability index. Almost all items had infit and outfit mean squares in the accepted range. CONCLUSION: Serbian version of the GQL-15 demonstrates adequate reliability and validity. This version of the GQL-15 is a valid instrument for evaluation of quality of life among Serbian speaking patients with glaucoma and can be applied in daily clinical work.

AB  - AIM: To evaluate glutathione transferase theta 1 and mu 1 (GSTT1 and GSTM1) polymorphisms as determinants of primary open angle glaucoma (POAG) risk, independently or in combination with cigarette smoking, hypertension and diabetes mellitus. METHODS: A case-control study with 102 POAG patients and 202 age and gender-matched controls was carried out. Multiplex-polymerase chain reaction method was used for the analysis of GSTM1 and GSTT1 polymorphisms. The differences between two groups were tested by the t-test or chi(2) test. Logistic regression analysis was used for assessing the risk for disease development. RESULTS: The presence of GSTM1-null genotype did not contribute independently towards the risk of POAG. However, individuals with GSTT1-active genotype were at almost two-fold increased risk to develop glaucoma (P=0.044) which increased up to 4.36 when combined with GSTM1-null carriers (P=0.024). When glutathione transferase (GST) genotypes were analyzed in association with cigarette smoking, hypertension and diabetes, only carriers of GSTT1-active genotype had significantly increased risk of POAG development in comparison with GSTT1-null genotype individuals with no history of smoking, hypertension and diabetes, respectively (OR=3.52, P=0.003; OR=10.02, P lt 0.001; OR=4.53, P=0.002). CONCLUSION: The results obtained indicate that both GSTM1-null and GSTT1-active genotypes are associated with increased POAG risk among smokers, suggesting potential gene-environment interaction in glaucoma development.

AB  - Purpose. To evaluate the effect of applied suction during microkeratome-assisted laser in situ keratomileusis (LASIK) procedure on peripapillary retinal nerve fiber layer (RNFL) thickness as well as macular ganglion cell-inner plexiform layer (GC-IPL) thickness. Methods. 89 patients (124 eyes) with established myopia range from -3.0 to -8.0 diopters and no associated ocular diseases were included in this study. RNFL and GC-IPL thickness measurements were performed by spectral domain optical coherence tomography (SD OCT) one day before LASIK and at 1 and 6 months postoperatively. Results. Mean RNFL thickness prior to LASIK was 93.86 +/- 12.17 mu m while the first month and the sixth month postoperatively were 94.01 +/- 12.04 mu m and 94.46 +/- 12.27 mu m, respectively. Comparing results, there is no significant difference between baseline, one month, and six months postoperatively for mean RNFL (p > 0.05). Mean GC-IPL thickness was 81.70 +/- 7.47 mu m preoperatively with no significant difference during the follow-up period (82.03 +/- 7.69 mu m versus 81.84 +/- 7.64 mu m; p > 0.05). Conclusion. RNFL and GC-IPL complex thickness remained unaffected following LASIK intervention.

Estimation of loneliness in students with visual impairments

AB  - Background/Aim. Loneliness is becoming more frequent, especially in young people. Some authors believe that visual impairments increase the risk of loneliness. Empirical data on its manifestation in persons with visual impairments are contradictory. The aim of this research was to determine the degree of loneliness in students with visual impairments and their peers from general population. Methods. A comparative research was conducted on a sample consisting of 36 visually impaired students and 101 students without visual impairments (control group). Students with visual impairments were divided into 3 subgroups with regard to the degree of visual impairments (students with low vision, legally blind, and totally blind students). University of California Los Angeles Loneliness Scale (which assesses the general loneliness) and Social and Emotional Loneliness Scale (which assesses social loneliness, family loneliness, and romantic loneliness) were used in our investigation. Results. The results showed that the general loneliness was significantly lower in the group of students with visual impairments than in the control group (t = 2.121; p = 0.036). There were no significant differences in the Social and Emotional Loneliness Scale between the group with visual impairments and the control group. No significant differences were determined in the level of loneliness among students with a different degree of visual impairments. There are significant differences in the manifestation level of social loneliness, family loneliness, and romantic loneliness (Wilk's lambda = 0.604; p ≤ 0.000) in the group of students with visual impairments. Romantic loneliness was manifested the most, followed by social loneliness, while family loneliness was the least manifested. Conclusion. With regard to the results of our research, visual impairment alone is not a crucial factor of loneliness in students with visual impairments. Further studies on protective factors of loneliness can contribute to its prevention in young people with visual impairments.
AB  - Uvod/Cilj. Usamljenost je sve učestalija, posebno u populaciji mladih. Neki autori smatraju da oštećenje vida povećava rizik od usamljenosti. Empirijski podaci o ispoljenosti kod osoba sa oštećenjem vida su kontradiktorni. Cilj našeg istraživanja bio je utvrđivanje stepena usamljenosti kod studenata sa oštećenjem vida i njihovih vršnjaka iz opšte populacije. Metode. Komparativno istraživanje je obavljeno na uzorku koji je činilo 36 studenata sa oštećenjem vida i 101 studenata bez oštećenja vida (kontrolna grupa). Na osnovu stepena oštećenja vida, studenti sa oštećenjem vida bili su podeljeni u tri podgrupe (slabovidi, praktično slepi i potpuno slepi). U našem istraživanju korišćeni su University of California Los Angeles Loneliness Scale (za procenu opšte usamljenosti) i Skala socijalne i emocionalne usamljenosti (za procenu socijalne, porodične i usamljenosti u ljubavi). Rezultati. Analiza rezultata je pokazala da je opšta usamljenost bila značajno manja u grupi studenata sa oštećenjem vida nego u kontrolnoj grupi (t = 2.121; p = 0.036). Na Skali socijalne i emocionalne usamljenosti nije utvrđena statistički značajna razlika između grupe studenata sa oštećenjem vida i kontrolne grupe. Takođe, nije utvrđena značajna razlika u nivou usamljenosti između studenata sa različitim stepenom oštećenja vida. U grupi studenata sa oštećenjem vida postojala je značajna razlika u stepenu ispoljenosti socijalne usamljenosti, usamljenosti u porodici i usamljenosti u ljubavi (Wilk's lambda = 0.604; p ≤ 0.000). Najviše je bilo ispoljena usamljenost u ljubavi, sledi socijalna usamljenost, dok je usamljenost u porodici bila najmanja. Zaključak. Prema rezultatima našeg istraživanja, samo oštećenje vida nije presudni činilac usamljenosti kod studenata sa oštećenjem vida. Buduća istraživanja protektivnih faktora usamljenosti mogu doprineti prevenciji ove pojave kod mladih sa oštećenjem vida.
