Specific surgery method for treatment of patients with cerebral palsy

AB  - The aim of this research was to evaluate the efficacy of the specific surgery method, called Ulzibat® surgery method, in the rehabilitation of children with cerebral palsy. 83 patients, aged 2.6 to 13.4yearswith spastic cerebral palsy participated in this study The average course of treatment was 2 stages. Thefollow-up period was 2-5years. GMFM-88 was used to assess the degree of motor disorders before and after treatment. Patients before and after surgical treatment were divided into 5 groups of levels of motor activity. Results showed that, after treatment, there was a significant increase in total score in average by 23 points in all observed group. There were a positive correlation between the difference in scores before and after treatment and the level of motor skills (p<0.05) and a negative correlation with age of operation (p<0.05). Most children after surgical treatment moved to a higher level of physical activity. The number of children with minimal disruption in motor activity (1st group) after treatment has doubled, and the number of children with the most severe disorders (4th and 5th group) was halved. Most of the children showed improvement either within their group movement activity (39%) or transition on level 1 above (49%). 12% of patients were passed on 2-4 levels above. It can be said, that this surgery method has shown high efficiency in the rehabilitation of children with cerebral palsy with different the severity of the disease. The most pronounced effect noted in the course of surgical treatment atan early age (3-7years).

AB  - Writing ability is a complex that includes a variety of skills and activities that must be in writing in the interaction. Successful writing is achieved only if all the skills and all of these processes work, and if in the course of learning, or acquisition of writing skills reach their full integrativity. Cerebral palsy does not have a typical clinical picture, but it is specific to each individual in relation to: the etiology of brain damage, localization and size of defects, the period in which the damage occurred. Given this fact assessment of a child with cerebral palsy should be based on the model estimates the potential and profile through achievements in certain areas, the qualitative and quantitative interpretation. The aim of this study is to assess the writing skills of boy who had undergone surgery of stage fibrotomy. The research results indicate that there is a visible improvement in writing skills after the completion of stage fibrotomy intervention (especially after the third intervention).
AB  - Pisanje je kompleksna sposobnost koja uključuje niz različitih sposobnosti i aktivnosti koje tokom pisanja moraju biti u interakciji. Uspešno pisanje se ostvaruje samo ako sve sposobnosti i svi ovi procesi funkcionišu i ako se tokom učenja, odnosno sticanja sposobnosti pisanja postigne njihova potpuna integrativnost. Cerebralna paraliza nema jednu karakterističnu kliničku sliku, već je ona specifična za svakog pojedinca u odnosu na: etiologiju oštećenja mozga, lokalizaciju i veličinu oštećenja, period u kojem je došlo do oštećenja. Obzirom na ovu činjenicu procenu deteta sa cerebralnom paralizom treba zasnivati na modelu procene potencijala, odnosno kroz profile postignuća u pojedinim područjima, kroz kvalitativnu i kvantitativnu interpretaciju. Cilj ovog rada je procena sposobnosti pisanja dečaka koji je bio podvrgnut hirurškoj intervenciji etapne fibrotomije. Rezultati istraživanja ukazuju na to da postoji vidljiv napredak u sposobnosti pisanja nakon izvršenih intervencija etapne fibrotomije (naročito posle treće intervencije).

AB  - The aim of this paper is to show the effects of surgical method of phase fibrotomy, developed and promoted by the Russian professor V.B. Uljzibat, on quality of life, the degree of functional dependence, and psychomotor performance in a patient with cerebral palsy through perceptive skills. Functional status was assessed before and after surgery using a battery of appropriate instruments of clinical assessment. Areas where most progress was achieved were the satisfaction of personal needs and mobility. The level of psychological organization was improved in all considered aspects. The achieved therapeutic effects justify the application of this method with precisely defined indications and implementation of further, more detailed and extensive clinical research in this area.
AB  - Cilj ovog rada je da se prikaže uticaj hirurške metode fazne fibrotomije, koja je razvijena i unapređena od strane ruskog profesora dr V.B. Uljzibata ,na kvalitet života, stepen funkcionalne zavisnosti, i psihomotorne sposobnosti bolesnika sa cerebralnom paralizom kroz grafomotorne veštine. Funkcionalni status je procenjivan pre i posle operacije korišćenjem baterije odgovarajućih instrumenata kliničke procene. Oblasti u kojima je bio postignut najveći napredak su zadovoljenje ličnih potreba i pokretljivost. Nivo psihološke organizacije je bio poboljšan u svim posmatranim aspektima. Ostvareni terapijski efekti opravdavaju primenu ove metode kod precizno utvrđenih indikacija kao i sprovođenje daljih, detaljnijih i opsežnijih kliničkih istraživanja u ovoj oblasti.
