Indicators of cognitive decline in persons with presbycusis

AB  - Introduction. Presbycusis or age-related hearing loss is a physiological elevation of hearing threshold and is one of the leading
chronic health problems. A growing number of studies show a
correlation between hearing loss in old age and decline in cognitive function. Material and Methods. The Serbian version of
the Montreal Cognitive Assessment test was used, and the study
included 56 subjects with a confirmed diagnosis of presbycusis
of which 29 (51.8%) were hearing aid users. Results. The score
of cognitive function in the respondents was lower compared to
the results of previous research (mean = 19.07; standard deviation
= 5.03). Also, there were significant differences between the
scores of cognitive function in persons who used amplification
and persons who did not. There was no significant correlation
between gender, employment status, and the degree of hearing
impairment, while the correlation was significant between the
level of education, age, the length of hearing aid use, and the
number of hours per day a person used a hearing aid. Regression
analysis showed that about 88% of the variance of the dependent
variable could be explained by four variables: age, level of education, length of hearing aid use, and daily level of hearing aid
use. Conclusion. Persons with presbycusis from the territory of
the Autonomous Province of Vojvodina showed poor performance scores on the cognitive ability test compared to the results
of other researchers. The results showed that the length of hearing aid use and the daily level of hearing aid use significantly
affect the cognitive functions in persons with presbycusis.
AB  - Uvod. Staračka nagluvost podrazumeva fiziološko povećanje
praga sluha i jedan je od vodećih hroničnih zdravstvenih problema. Sve veći broj istraživanja pokazuje međusobnu povezanost gubitka sluha u starosti i pada kognitivne funkcije. Materijal i metode. Korišćena je srpska verzija Montrealske procene kognicije, a studija je obuhvatila 56 ispitanika sa potvrđenom
dijagnozom prezbiakuzije, od čega je 29 (51,8%) korisnika slušnih aparata. Rezultati. Skor kognitivnog funkcionisanja ispitanika bio je niži u odnosu na rezultate prethodnih istraživanja
(srednja vrednost = 19,07; standradna devijacija = 5,03). Postoje značajne razlike između skora kognitivnog funkcionisanja
osoba koje koriste amplifikaciju i osoba koje je ne koriste. Nema
značajne korelacije sa polom i statusom zaposlenja, kao ni stepenom oštećenja sluha. Korelacija je značajna sa stepenom
obrazovanja, starošću, dužinom korišćenja slušnog aparata, kao
i brojem sati u danu u kom osoba koristi slušni aparat. Regresionom analizom utvrđeno je da oko 88% varijanse zavisne
promenljive objašnjavaju četiri varijable – starost, stepen obrazovanja, dužina korišćenja i dnevni nivo korišćenja slušnog
aparata. Zaključak. Osobe sa prezbiakuzijom sa teritorije Autonomne Pokrajne Vojvodine pokazale su prosečno niže postignuće na testu kognitivnih sposobnosti u odnosu na rezultate
drugih istraživača. Rezultati su pokazali da dužina korišćenja
slušnog aparata, ali i korišćenja slušnog aparata na dnevnom
nivou, značajno utiču na kognitivne funkcije osoba sa prezbiakuzijom

AB  - Razumljivost govora gluve i nagluve dece je značajno narušena zbog
nepravilnog izgovora glasova. Nemogućnost da precizno čuju govorne obrasce iz
neposrednog okruženja ih sprečava u imitaciji a samim tim i u usvajanju govora i
jezika. Gluva i nagluva deca, u zavisnosti od stepena gubitka sluha, ne mogu
auditivno da kontrolišu svoj glas, što im dodatno pravi teškoću. Poslednjih decenija
u većini zemalja intezivno se razvijaju programi kohlearne implantacije koji
značajno doprinose povećanju perceptivnih mogućnosti dece oštećenog sluha, što
dovodi do bržeg i lakšeg učenja govora i jezika.
Cilj ovog rada je bio ispitivanje kvaliteta izgovora glasova kod dece sa
kohlearnim implantom, kao i ispitivanje uticaja nekih vremenskih činilaca. U
uzorku je bilo 25-oro dece oba pola, uzrasta od 3 do 10 godina. Govorna baza za
ispitivanje kvaliteta izgovora glasova formirana je korišćenjem Globalnog
artikulacionog testa autora Spasenije Vladisavljević. Ocena izgovora glasova
rađena je na dva nivoa. Prvi nivo procene rađen je od strane obučenog stručnjaka
koji je svoje ocene beležio u predviđeni formular. Drugi nivo procene rađen je
spektralnom analizom izgovorenih glasova, takođe od strane obučenog stručnjaka.
U obradi rezultata korišćene su metode deskriptivne statistike i jednosmerna
analiza varijanse (ANOVA).
Rezultati pokazuju da se u govoru kohlearno implantirane dece javljaju sva
tri tipa artikulacionih odstupanja (omisija, supstitucija i distorzija), da se
artikulacione greške javljaju u svim grupama glasova, najčešće u grupi afrikata i frikativa. Gluva deca koja su kohlearno implantirana na ranijem uzrastu i koja su
obuhvaćena odgovarajućim programima re/habilitacije postižu bolju auditivnu
kontrolu svog govora, normalizuju svoj osnovni glas i poboljšavaju svoje
artikulacione sposobnosti
AB  - Speech coherence of deaf and hard of hearing children is significantly
impaired due to irregular pronunciation of sounds. Inability to hear speech patterns from immediate environment accurately makes it impossible for them to imitate
and acquire speech and language. Depending on the degree of a hearing loss, deaf
and hard of hearing children cannot control their voice auditorily, which causes
additional problems. In the last few decades, cochlear implantation programs have
intensively been developing in most countries, which significantly influences the
increase in perceptive abilities of children with hearing impairments leading to
faster and easier learning of speech and language.
The aim of this paper was to test the quality of sound pronunciation in
children with cochlear implants, and to test the influence of time factors. The
sample consisted of 25 children of both genders, aged between 3 and 10. Speech
basis for testing the quality of sound pronunciation was formed by using the Global
Articulation Test by Spasenija Vladisavljevic. The assessment of sound
pronunciation was performed at two levels. The first level of assessment was
performed by a trained professional who noted his marks in the provided form. The
second level of assessment was performed by spectral analysis of the pronounced
sounds, also by a trained professional. Methods of descriptive statistics and one
way variance analysis (ANOVA) were used in results analysis.
The results indicate that all three types of articulation errors (omission,
substitution, and distortion) occur in the speech of children with cochlear implants,
and that articulation errors occur in all groups of sounds, usually in affricates and
fricatives. Deaf children who were implanted at an early age and are included in
appropriate rehabilitation programs have better auditory control of their speech,
normalize their voice and improve their articulation abilities.

Upotreba prostih i nezavisnih rečenica kod dece sa kohlearnim implantom

AB  - Ovim radom smo želeli da ispitamo sintaksičke konstrukcije kod
dece oštećenog sluha sa kohlearnim implantom koja se nalaze na re/
habilitacionom tretmanu.
Istraživanjem je obuhvaćeno 25 prelingvalno gluve dece sa kohlearnim
implantom i 25 dece sa urednim sluhom, uzrasta od 4 do
8 godina. Grupe su ujednačene prema polu, uzrastu, intelektualnim
sposobnostima, toku trudnoće i apgar skoru u prvom i petom minutu.
Pratili smo tok razvoja govora i korišćenja rečenica kod dece sa kohlearnim
implantom i poredili ih sa kontrolnom grupom dece koja imaju
uredan sluh.
Istraživanjem je potvrdjeno da produkcija prostih i nezavisnih
rečenica kao i usvajanje ovog segmenta jezičke strukture dece sa kohlearnim
implantom sledi tok urednog jezičkog razvoja. Međutim uočeno
je da postoji značajan zaostarak u odnosu na uzrast. Deca sa
kohlearnim implantom u prvoj godini rehabilitacije i slušanja pomoću
implanta nemaju prostu i nezavisnu recenicu. Prosta rečnica se pojavljuje
u toku druge, a nezavisna u toku treće godine slušne starosti
i rehabilitacije.
AB  - The aim of this paper is to test syntactic constructions in hearing
impaired children with cochlear implants undergoing a rehabilitation
treatment. The research included 25 prelingually deaf children with
cochlear implants and 25 children with regular hearing, aged between
4 and 8. The groups are equal in gender, age, intellectual abilities,
course of pregnancy and the Apgar score in the 1st and 5th minute. The
development of speech and use of sentences in children with cochlear
implants was observed and compared to the control group of children
with regular hearing.
The research confirmed that the production of simple sentences
and independent clauses and mastering this segment of language
structures follow normal language development in children with
cochlear implants. However, a significant delay was observed with
regard to age. Children with cochlear implants do not use simple
sentences and independent clauses in the first year of rehabilitation
and hearing with the aid of implants. A simple sentence occurs during
the second, and an independent clause during the third year of hearing
age and rehabilitation.

abstract = "Ovim radom smo želeli da ispitamo sintaksičke konstrukcije kod
dece oštećenog sluha sa kohlearnim implantom koja se nalaze na re/
habilitacionom tretmanu.
Istraživanjem je obuhvaćeno 25 prelingvalno gluve dece sa kohlearnim
implantom i 25 dece sa urednim sluhom, uzrasta od 4 do
8 godina. Grupe su ujednačene prema polu, uzrastu, intelektualnim
sposobnostima, toku trudnoće i apgar skoru u prvom i petom minutu.
Pratili smo tok razvoja govora i korišćenja rečenica kod dece sa kohlearnim
implantom i poredili ih sa kontrolnom grupom dece koja imaju
uredan sluh.
Istraživanjem je potvrdjeno da produkcija prostih i nezavisnih
rečenica kao i usvajanje ovog segmenta jezičke strukture dece sa kohlearnim
implantom sledi tok urednog jezičkog razvoja. Međutim uočeno
je da postoji značajan zaostarak u odnosu na uzrast. Deca sa
kohlearnim implantom u prvoj godini rehabilitacije i slušanja pomoću
implanta nemaju prostu i nezavisnu recenicu. Prosta rečnica se pojavljuje
u toku druge, a nezavisna u toku treće godine slušne starosti
i rehabilitacije., The aim of this paper is to test syntactic constructions in hearing
impaired children with cochlear implants undergoing a rehabilitation
treatment. The research included 25 prelingually deaf children with
cochlear implants and 25 children with regular hearing, aged between
4 and 8. The groups are equal in gender, age, intellectual abilities,
course of pregnancy and the Apgar score in the 1st and 5th minute. The
development of speech and use of sentences in children with cochlear
implants was observed and compared to the control group of children
with regular hearing.
The research confirmed that the production of simple sentences
and independent clauses and mastering this segment of language
structures follow normal language development in children with
cochlear implants. However, a significant delay was observed with
regard to age. Children with cochlear implants do not use simple
sentences and independent clauses in the first year of rehabilitation
and hearing with the aid of implants. A simple sentence occurs during
the second, and an independent clause during the third year of hearing
age and rehabilitation.",

AB  - Kao rezultat sve većih očekivanja i zahteva korisnika asistivnih tehnologija u cilju povećanja njihovih sposobnosti i postizanja većeg ste- pena samostalnosti i nezavisnosti u funkcionisanju, dolazi do sve inten- zivnijer razvoja i primene asistivnih tehnologija. Na obaveznost i neop- hodnost primene i ovog vida podrške i pomoći osobama sa različitim vi- dovima invaliditeta posebno su ukazala različita zakonska akta i kla- sifikacije. Ukazano je i na neka pitanja koja se u radu diskutuju kao što su: faktori koji utiču na izbor pomoćnih tehnologija, korisnost primene i barijere za njihovu primenu. Na rezultate korišćenja uticaće kako sre- dinski, tako i lični faktori, odlike samog proizvoda i pravilna procenu potreba korisnika. Koristi primene ovih tehnologija su brojne, ali je neo- phodan i stručan, profesionalan pristup prilikom njihovog odabira i rada sa korisnicima i njihovim porodicama
AB  - Due to growing expectations and demands from users of assistive
technologies, and with the aim of improving their abilities and ensuring a
higher degree of independance and self-reliance in their functioning, there
have been major advances in the development and application of assistive
technologies. The requirement and need for providing this type of support
and help to people with different types of disabilities have been highlighted
in a series of legislative acts and classifications. They raise some questions
addressed in this paper such as: factors that influence assistive technology
selection, benefits from and obstacles to their application. The outcomes
of using these technologies will be influenced by environmental as well as
personal factors, features of the product itself, and proper assessment of
user needs. There are numerous benefits of applying these technologies;
however, a professional approach during the entire selection process and
while working with users and their families is required.
